Yes, I currently accept Blue Cross Blue Shield of MN (medicaid, medicare and commercial), Aetna, and Cigna insurances.
If I don’t accept your insurance, we can navigate this. I am happy to provide you with a form you can send to your insurance for possible reimbursement. That means you’re responsible for the fees at the time of our appointment and depending on your plan, a certain percentage of that may be paid to you by your insurance company. I encourage you to call the number on the back of your insurance card to find out what your “out-of-network” benefits are just so there aren’t any scary financial surprises. Ask them where to send what’s called a “Superbill,” that’s the form I’ll give you.
I accept cash, credit cards, Venmo, or HSA/FSA (health savings account) cards.
Yes. I do offer adjusted fee rates and payment plan options. Please reach out with any questions.
I do my best to respond to booking requests and any additional questions within 48 hours.
Deeply Rooted will provide you with an intake packet that you can fill out and submit online (via your phone, tablet, or computer). An initial assessment is completed during the first appointment to create a plan designed by and for you specifically.
Deeply Rooted offers individual, couples, and family sessions in-person, virtually, or outdoors/in nature (weather and distance permitting). If we will be meeting virtually, a link will be shared by email and/or text message after you are scheduled.
At this time, this can be decided according to your comfort level. Masks are not mandatory for in-person appointments.
Deeply Rooted is located on the second floor of the Duluth Folk School and the Dovetail Cafe at 1917 West Superior St. in Duluth, MN. When you come in the front door of the folk school, walk straight back past the cafe and marketplace. Take the staircase on the left up to the second floor (there's only one staircase). My office is straight ahead. Look for the sign above my yellow door. If I am in session, feel free to have a seat at the table and I will be right with you! If my door is open or the sign says, "Welcome, Please Knock", please do! ***At this time there is no wheelchair access to the second floor.
